PHLEBOTOMY CLASS INFORMATION SHEET
A phlebotomist is a medical technician who draws blood for analysis.
- Copy of High School Diploma/Transcript or GED/TASC & copy of Photo I.D.
- $30 non-refundable Application fee- Paid by Entrance Exam date
- Entrance Exam – TABE Test (Reading and Math)
- Driver's License or Photo ID
- Proof of Insurance
- Agreement to submit to a drug screen and criminal background check before clinical experience.
- $170 seat deposit must be paid no later than July 19, 2019
- Minimum 10 students registered and enrolled for class to run.

Entrance Exam Date: Date and time TBD- Students who have turned in applications before May 1st will be contacted for a test date and time between mid- May & early June.

Deadline to register: July 19, 2019
Class Starts: August 12, 2019 at 5:30 p.m.
Class meets: Two nights a week, Mondays/Wednesdays – Approximately 12 weeks – 5:30 p.m. to 8:15 p.m.
174 hours including clinical hours/Daytime hours may be required for the clinical experience.

Photo ID required to be admitted into test, receipt from exam payment, #2 pencils and a calculator.
Admission to the Phlebotomy class is based on test results as compared to other candidate’s courses. If accepted, the instructor will conduct interviews to clarify requirements of the course.
Class size is limited!!
Available to those who qualify. FASFA must be completed for 2019-2020 school year at FAFSA.gov
The Financial Aid Specialist is available on Wednesdays and Thursdays from 9:00 a.m. to 1:00 p.m. in the main office.
Part Time financial aid funding is allotted by HEPC to the fall class and the spring class, divided equally among all eligible students. Financial aid available will not cover all program costs.
A Payment Plan will be set up for students to pay amounts of program costs not covered by the HEAPs Grant Financial Aid
